Agency Forms
Patriot Act
In order to process the dollars that have been directed towards your agency, we will need you to comply with the Patriot Act. If you haven't already completed the form for the 2011-12 campaign year, print a copy, then complete, sign and upload the form to us through your Agency Portal login. Once we have verified this form, your funds will be released.

Electronic Payments
In order to process your payments electronically, we will need a completed ACH Information Form. Print a copy of the form, complete and sign, then upload it with a voided check through your Agency Portal login. Once we have verified these forms, your payments will be deposited into the banking account you have provided. Click here to download the ACH form.
